Items needed for Football-4-Life registration:

Copy of birth certificate
Photo ID Card (School ID, State issued)
Copy of last report card
Copy of last physical ( if previous year 1 must be scheduled in the next 60 days)

Items needed for Football-4-Life Camp:

2 pair sweat pants 1 black – 1 blue
1 pair of black and white football cleats
2 short sleeve football shirts – 1 black & 1 white (Provided By Football-4-Life)
2 long sleeve football shirts 1 black – 1 white (Provided By Football-4-Life)
2 pair of sweat shorts 1 black – 1 blue (Provided By Football-4-Life)
